I’d set off, with my friend Matthew Price, to sail down a little-known river in the rainforest, in a homemade boat. Welcome to the Netherlands, a tiny country that only extends, at its broadest, 312 km north to south, and 264 km east to west - despite the land area increases slightly each year as a result of continuous land reclamation and drainage. Large animals that inhabit the desert have evolved a number of adaptations for reducing the effects of extreme heat. One adaptation is to be light in color, and to reflect rather than absorb the Sun's rays. Desert mammals also depart from the normal mammalian practice of maintaining a constant body temperature. Instead of trying to keep down the body temperature deep inside the body, which would involve the expenditure of water and energy, desert mammals allow their temperatures to rise to what would normally be fever height, and temperatures as high as 46 degrees Celsius have been measured in Grant's gazelles. The overheated body then cools down during the cold desert night, and indeed the temperature may fall unusually low by dawn, as low as 34 degrees Celsius in the camel. This is an advantage since the heat of the first few hours of daylight is absorbed in warming up the body, and an excessive build-up of heat does not begin until well into the day.
Another strategy of large desert animals is to tolerate the loss of body water to a point that would be fatal for non-adapted animals. The camel can lose up to 30 percent of its body weight as water without harm to itself, whereas human beings die after losing only 12 to 13 percent of their body weight. An equally important adaptation is the ability to replenish this water loss at one drink. Desert animals can drink prodigious volumes in a short time, and camels have been known to imbibe over 100 liters in a few minutes. A very dehydrated person, on the other hand, cannot drink enough water to rehydrate at one session, because the human stomach is not sufficiently big and because a too rapid dilution of the body fluids causes death from water intoxication.
The tolerance of water loss is of obvious advantage in the desert, as animals do not have to remain near a water hole but can obtain food from grazing sparse and far-flung pastures. Desert-adapted mammals have the further ability to feed normally when extremely dehydrated, it is a common experience in people that appetite is lost even under conditions of moderate thirst.

A. The medical profession is currently under siege as never before with a spate of high profile malpractice cases. This attack is taking place at a time when the National Health Service undergoing a “culture change” brought about by a shift in the public’s attitudes to authority, in general, and, more specifically, by the demystification of medicine. The perception that doctors are a race apart is finally beginning to wane.
B. These forces have, fortunately, already led to a number of radical developments in the last five or six years in the way doctors are being trained, with greater emphasis now being laid on a more patient-oriented approach. Whilst, in the past, communicating effectively with patients was left basically to chance, this is no longer the case. As part of their final assessment, doctors now have to take a practical examination where their communication as well as clinical skills are carefully scrutinized.
C. If you ask most people what makes a good doctor, they will not say someone with sound medical knowledge. The first thing that will spring to mind is a good bedside manner; in other words, good communication skills. But what does a good bedside manner, or communication skills, entail?
D. All too often people complain about the lack of sensitivity of the doctors they encounter whether they be generalists or specialists. Some other frequently voiced criticisms are that doctors sound as if they are delivering a lecture when talking to patients; pontificating from on high. Or that they lack basic social skills; or indeed that they are bad listeners, concerned only with delivering their message rather than becoming involved with any kind of negotiation with the patient. So it would be safe to say that the most important aspect of a good bedside manner is good interpersonal skills.
E. From the patients’ point of view, the interaction they have during their consultation with a doctor is very personal and hence emotional, while for the doctor it is merely a logical and objective process. And so, the chances of the doctor/patient communication breaking down are high if the doctor is not sufficiently skilled in handling the patient’s emotional needs. A doctor must be able to deal with the full range of a patient’s feelings showing sympathy and empathy especially when handling difficult situations, like breaking bad news, etc.
F. Another aspect of the good bedside manner, which is more often than not overlooked, is having the ability to talk to patients using lay language that they understand, while at the same time, avoiding any hint of condescension, or being patronising. The inability to do this has a number ofeffects. When doctors use medical jargon, patients feel that they are trying to hide something. Doctors can also give the impression that they do not know what they are talking about, or even that they do not know the solution to a problem.
G. It is also essential that the doctor at all times is able to maintain authority. For example, the doctors need to deal with some patients’ belief that medicine is infallible, i.e. that the doctor has the panacea for every woe! This is certainly no easy task, as most people ‘s expectations are raised by the daily diet of wondrous developments in medicine.
H. The other side of the coin is that, as people’s awareness and knowledge have increased, albeit often misinformed by the internet, etc., the stronger their doubts about the medical profession have become. And coupled with the rise in general educational awareness, the public have consequently a lower regard for doctors. This, in turn, has affected the doctor’s ability to communicate. They are not able to hide behind the veneer that technical jargon created.
I. At last, the pendulum has swung in the patient’s direction. The onus is now upon doctors to adapt themselves to the patient’s needs rather than the patient approaching some awesome god-like figure. The veil has been lifted and the temple violated.

MUSIC REVIEWS: ARTISTS and ALBUMS
A. Think Of One: Camping Shaabi
Think Of One is truly unique. Over the years, this Antwerp- based group have worked and recorded with a wide range of artists such as Afro-Brazilian percussionists and Inuit throat singers, but for this album, they return to Moroccan themes. The Moroccan effect is apparent straight away in the spellbinding rhythms and voices of the first track, 'J'étais Jetée'. And that's just for starters - the recording goes on to mix diverse sounds and types of music at an astounding speed. The quality doesn't falter from one track to the next and each track is innovative in its own way. The vintage keyboards and Balkan-style brass section are always there, laying the foundations for the other sounds which are brought in and used around them. In a dazzling combination of Flemish, French, Arabic and English, the band's lyrics also fascinate, some having a serious tone and others being more frivolous, but a singable tune always surfaces.
B. Neco Novellas: Khu Kata
Neco Novellas is a singer-songwriter with immense talent and imposing stage presence. His new album, 'Khu Kata', presents influences of his teenage years in Mozambique. Guest vocalist Lilian Vieira of Zuco 103 enriches the track called 'Vermeiha' which is a successful mix of Brazilian samba and Mozambican pop, while the track called 'Zula Zula' really shows what Novellas can do. But with 'Phumela' things slidedownhill for a while, and the lyrics of 'Swile Navo' can only be described as banal. He returns to form with 'The Train’, which is beautifully arranged and owes an obvious debt to the Hugh Masekeia songbook ('Stimela!'), but the best tracks are the uplifting 'Tikona' and '0 Sol', which truly stand out as the blend of world/jazz fusion that this artis regularly delivers. Nonetheless, 'Khu Kata' would have been improved by more rigorous editing and slightly fewer tracks.
C. Various artists: Nigeria Disco Funk Special
In the 1970s, Lagos was a creative place musically, and would-be artists flocked there from all over Africa to put their very individual spin on imported music. The first number, an instrumental by Sahara All Stars entitled 'Take Your Soul', is bravely funky and strikes just the right opening note. The next outstanding track is by the talented Johnny Haastrup, who gives a great rendition of 'Greetings'. It is hard to understand why he never really made it as a soloist, because this piece is both harmonic and flamboyant. Sadly, the remaining songs are not in the same league, and you may be disappointed that they lack a truly authentic and traditional feel. Also, the material is mostly instrumental, so there are few singing stars in evidence. But despite this, the album is well worth listening to. It's arranged in an order suitable for clubs, which is doubtless what the artists originally intended.
D. WatCha Clan: Diaspora Hi-Fi - A Mediterranean Caravan
The album begins with a scrambled montage of voices and Arabic strings and percussion; right away, the listener is projected into a mix of dance floor sounds. This is fusion music, dubbed and electroed. Watcha Clan put forward a dilemma: can diverse influences result in a harmonious whole? Or does it just end up as a mish-mash of indistinguishable sound? They certainly add a rich variety of flavours to the dominant rhythms. Some people can't get enough of vocalist Siesta K's unusual voice, but for others even a little is too much. Nassim Kouti sometimes accompanies her on vocals and guitar. One of these tracks is the haunting 'Chiclet Layna'. The beginning of ‘Oued El Chotili’ is equally tranquil and briefly entrances before the reggae beat takes over, powered by Moroccan castanets. The song stands out on the album because the really impressive combination of styles works so well. This doesn't always happen on other tracks, but watching the bonus video makes you realise what this band is all about and that they had a lot of fun making this album.
